Family camp reservations at Ponderosa Lodge

On behalf of _____________________, I am requesting reservations at Ponderosa Lodge for exclusive use of the lodge and cabins, for ___ one-week family camp(s). The first week is scheduled to arrive for dinner on _____________, 200__ and depart after breakfast _____________, 200__. If more than one week is requested, the second week is scheduled to arrive for dinner on _____________, 200__ and depart after breakfast _____________, 200__.

I understand and agree:

1. That space for this group is not guaranteed until this completed form is returned to the Ponderosa and the $4,000 deposit for each week is received.

2. That the rate for lodging, breakfast, lunch and dinner is calculated in accordance with the rate table below. Ponderosa Lodge’s charge is not decreased if any, or all, guests arrive late, depart early or miss any meal(s) or other service.

c. That the minimum charge for each week is $16,000 regardless of amount collected for the other week. For example, if the totals for the two weeks separately amount to $25,000 and $12,000, Ponderosa Lodge will collect $41,000, based upon the actual amount for first week and $16,000 for the second week.

d. That Ponderosa Lodge will collect 6% tax in addition to the rates quoted, unless a tax exemption affidavit is completed.

e. That lodging without meals for the Saturday night between consecutive weeks may be obtained for an additional charge of $25 per adult and $15 per child.
